용량

보통 성인에게 트렐라그립틴으로서 100mg을 주1회 동일한 요일에 경구 투여한다.
이 약의 분할선은 용량 조절을 위한 것이 아니다. 정제를 분할하거나 분쇄하여 투여하지 않는다.

용법

특수 환자군에서의 용법·용량
1. 고령자
고령자는 일반적으로 신기능이 저하되어 있는 경우가 많으므로 이상반응의 발현에 주의하고, 환자의 상태를 충분히 관찰하면서 신중히 투여한다.
2. 소아
저체중출생아, 신생아, 영아, 유아 및 소아에 대한 안전성은 확립되어 있지 않다(임상 경험이 없다).
3. 신기능 장애
중등도의 신기능 장애 환자에서는 배설 지연으로 인해 트렐라그립틴의 혈중농도가 상승할 수 있으므로 100 mg 투여는 권장되지 않는다.
- 중증의 신기능 장애 환자 또는 투석 중인 말기 신부전 환자에는 투여하지 않는다.
- 경증의 신기능 장애 환자에서는 용량 조절이 필요하지 않다.
4. 간기능 장애
간기능 장애 환자에서는 용량 조절이 필요하지 않다.

자파텍정100mg 부작용·주의사항

1. 저혈당

트렐라그립틴과 설포닐우레아계 약물 또는 인슐린 제제를 병용 투여하는 경우 저혈당의 위험이 증가할 수 있다. 따라서 트렐라그립틴과 병용 시 저혈당 발생 위험을 감소시키기 위하여 설포닐우레아계 약물 또는 인슐린 제제의 용량 감소를 고려하여야 한다.

2. 급성 췌장염

트렐라그립틴 및 다른 DPP-4 저해제에서 시판 후 급성 췌장염이 보고된 바 있다. 트렐라그립틴 투여 시작 후에는 췌장염의 징후 및 증상에 대하여 환자를 주의 깊게 관찰하여야 한다. 췌장염이 의심되는 경우에는 이 약의 투여를 즉시 중단하고 적절한 처치를 시행하여야 한다.

Metoprolol succinate extended-release tablets are a beta-adrenergic blocker indicated for the treatment of: • Hypertension, to lower blood pressure. Lowering blood pressure reduces the risk of fatal and non-fatal cardiovascular events, primarily strokes and myocardial infarctions. (1.1) • Angina Pectoris (1.2) • Heart Failure, to reduce the risk of cardiovascular mortality and heart failure hospitalizations in patients with heart failure. (1.3) 1.1 Hypertension Metoprolol succinate extended-release tablets are indicated for the treatment of hypertension, to lower blood pressure. Lowering blood pressure lowers the risk of fatal and non-fatal cardiovascular events, primarily strokes and myocardial infarctions. These benefits have been seen in controlled trials of antihypertensive drugs from a wide variety of pharmacologic classes including metoprolol. Control of high blood pressure should be part of comprehensive cardiovascular risk management, including, as appropriate, lipid control, diabetes management, antithrombotic therapy, smoking cessation, exercise, and limited sodium intake. Many patients will require more than 1 drug to achieve blood pressure goals. For specific advice on goals and management, see published guidelines, such as those of the National High Blood Pressure Education Program’s Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC). Metoprolol succinate extended-release tablets are contraindicated in severe bradycardia, second or third degree heart block, cardiogenic shock, decompensated heart failure, sick sinus syndrome (unless a permanent pacemaker is in place), and in patients who are hypersensitive to any component of this product. • Known hypersensitivity to product components. (4) • Severe bradycardia. Greater than first degree heart block, or sick sinus syndrome without a pacemaker. (4) • Cardiogenic shock or decompensated heart failure. (4)

• Abrupt cessation may exacerbate myocardial ischemia. (5.1) • Heart Failure: Worsening cardiac failure may occur. (5.2) • Bronchospastic Disease: Avoid beta-blockers. (5.3) • Concomitant use of glycosides, clonidine, diltiazem and verapamil with beta-blockers can increase the risk of bradycardia. (5.4) • Pheochromocytoma: Initiate therapy with an alpha blocker. (5.7) • Thyrotoxicosis: Abrupt withdrawal in patients with thyrotoxicosis might precipitate a thyroid storm. (5.8) • Peripheral Vascular Disease: Can aggravate symptoms of arterial insufficiency. (5.9) • Patients may be unresponsive to the usual doses of epinephrine used to treat allergic reaction. (5.10) 5.1 Abrupt Cessation of Therapy Following abrupt cessation of therapy with certain beta-blocking agents, exacerbations of angina pectoris and, in some cases, myocardial infarction have occurred. When discontinuing chronically administered metoprolol succinate extended-release tablets, particularly in patients with ischemic heart disease, gradually reduce the dosage over a period of 1 to 2 weeks and monitor the patient.
